The Residential Case Manager will be responsible for the assessment and treatment planning of all residents living in the Residential Program at Bridge. The residential program is a program for homeless youth 18-24 years old who can commit to counseling, maintain employment, continue their education, and work to obtain long-term transitional or permanent housing. The Residential Case Manager will meet with each youth weekly to develop and monitor progress towards meeting their goals.
KEY RESPONSIBILITIES:
Meet with each youth once per week and document this meeting with a clinical note in ETO, Bridge’s electronic record system.
Provide assessment of Resident’s behavioral health needs (i.e., depression, anxiety, trauma, substance use) and make referrals to appropriate services.
Utilize Motivational Interviewing (MI) and Collaborative Problem Solving (CPS) as primary interventions to engage youth around goals and encourage further involvement in Bridge services.
Promptly address Residents struggles to meet program expectations (i.e., not engaging in employment, not following through on services, displaying problematic substance use).
Complete ETO documentation to enroll youth when they enter the program, dismiss youth when they leave, and complete HMIS (entrance/annual/exit) assessments for each youth.
QUALIFICATIONS :
2-4 years of experience working with youth and young adults ages 18-24 in one or more of the following areas: behavioral health, housing management, job development, community relations, substance abuse or recreational counseling. Can also include supervised internship experience.
Ability to identify and execute effective problem-solving strategies. Must possess sound crisis intervention and conflict resolution skills.
A Bachelor's Degree in Human Services, Social Work or any other related degree, a Master Degree in Social Work or related field strongly preferred.
Comfort and confidence working in a fast-paced, dynamic work environment.
Passionate about working with homeless transitional youth population.
