Position Description
Position: On Site/Live-in Residential Services Case Manager I
Reports To: Residential Services Program Coordinator
Status: Full Time Hours/week: 40 Type: Non-exempt
Salary Range: $21.50 per Hour
Position Summary:
Accountable for the delivery of high-quality residential rehabilitation and case management services to the mentally ill adults living at Villa Calleguas. Assures timely completion of all documentation as required by HUD and local housing authorities. Ensures communication with collateral agencies and providers. Lives on-site and allows access to facilities for inspections and maintenance. Reports any observed violations of lease and/or house rules to Program Coordinator. Participates as a member of Turning Point Foundation’s Residential Services Team.
ACCOUNTABILITIES
1. Assure, in collaboration with Residential Services Coordinator, implementation of the residential services program for a defined resident population. This includes but is not limited to:
a. Assists Residential Services Coordinator in the recruitment, screening, and selection of appropriate residents.
b. Maintain regular personal contact with residents to monitor physical and emotional capacity for independent living and the need for additional services.
c. Coordinate support as necessary, making referrals to medical, mental health, substance abuse, and/or other necessary services. Regular communication with collateral service providers to ensure continuity of care.
d. Formulate needs assessments and oversee service plans in conjunction with collateral treatment providers.
e. With Residential Services Coordinator, develop and maintain community resource guide to serve as reference guides to residents and staff.
2. Assure, in collaboration with Residential Services Coordinator and Administrative staff, compliance with property management aspects of transitional housing for residents of the program. This may include but is not limited to:
a. Notifying Program Manager of any observed maintenance and repair needs of the facility.
b. Being available, within scheduled hours, to allow supervised entry into facility or grounds for authorized repairs, maintenance, and site visits.
c. Completion of rent-up documentation including verification of eligibility, income, and lease.
d. Provide information and data for reports as required.
3. Serve as an effective member of the Residential Service’s Team including but not limited to:
a. Ensure compliance with administrative policies and procedures.
b. Attend weekly individual supervision with Residential Services Coordinator and weekly house meetings.
c. Participate in other meetings, training and development as requested.
4. Other duties as assigned by supervisor.
QUALIFICATIONS
Education/Experience
BA degree and two (2) years experience in human services, one (1) year in residential services OR
Four (4) years of a*) experience providing direct client services to adults with severe and persistent mental illness preferably within the psychosocial rehabilitation model ANDb*) demonstrated ability to successfully conduct residential program implementation, record keeping and tracking, interagency coordination and administrative duties as assigned.
Demonstrated effectiveness in addressing complex consumer needs.
Computer proficiency in word processing and database management.
Strong written and oral communication skills.
Bilingual/bicultural skills desirable.
