Description: Salary Range: $37.50/hour-$48.50/hour Job Summary: Deliver telephone-based interventions promoting wellness, self care and chronic condition management using behavior-change approaches. As the recognized behavior change expert, act as a referral and resource for providers, support staff and others throughout the region. Offer members appropriately tailored resources and referrals within the KP system and outside community. Interact and collaborate with clinicians/health care team as needed to coordinate care/service and provide KPCO electronic medical records documentation. To provide trainings/education on the department and its programs and services, as well as behavior change principles and wellness topics to members, clinicians, medical office staff, other Kaiser Permanente employees, employer groups, and the community.
Essential Responsibilities: - Conduct client triage to assess and provide appropriate health education services, counsel and motivate clients appropriate to clients stage of readiness, encourage personal responsibility and reinforce self-care principles. Operate as part of a multidisciplinary team with health care team, pharmacy, etc. Document clients progress, outcomes, etc. in KPCO electronic medical records, and in department databases as needed. Refer appropriately.
- Interact with medical staff via KPCO electronic medical records referrals, phone, e-mail, and other means to provide consultation on clients health education needs.
- Assist in the development, implementation and evaluation of health education interventions, products and webinars. Review and give input on staff/member communication, policies, protocols, programs and services specific to the health behavior coach team.
- Facilitate health education interventions/trainings to individuals and groups within and outside of KPCO including but not limited to KPCO staff, members, employer groups and community groups.
- Represent department to KPCO staff, interregional committees and workgroups, employer groups, professional organizations, community health events and coalitions, worksites and promotional programs.
- Engage in activities to support department operations: participates in team and staff meetings and continuing education. Provide and participate in peer consult, support, and triage around Health Education Services interventions.
- Perform other duties which are appropriate to the position as requested.
Basic Qualifications: Experience
- Minimum two (2) years of (full time equivalent) experience with health behavior change/health coaching.
Education
- Bachelors degree in health education, education, social work, behavioral sciences, health sciences, nursing or related field required.
License, Certification, Registration
- Health and Wellness Coach Certificate within 12 months of hire
Additional Requirements: - Advanced knowledge of health behavior change theories and models, including motivational interviewing (showing minimum of 16 hours MI or other health behavior training, CHES certified or equivalent certification).
- Knowledge of adult learning theories and models.
- Demonstrated customer-focused service skills.
- Demonstrated ability to multi-task effectively.
- Ability to conduct real-time resource and information searches using Internet.
- Microsoft Office competency.
Preferred Qualifications: - Minimum two (2) years of experience with addicted populations (preferably tobacco-cessation).
- Minimum three (3) years of experience with health behavior change/health coaching.
- Minimum three (3) years of experience working in an HMO setting or other health care setting.
- Minimum one (1) year of program design, management and evaluation.
- Current certification in wellcoachesE or equivalent accredited program.
- Knowledge of best practices for tobacco-cessation.
- Microsoft Access or other database programs.
- Ability to use data from various sources to report on and inform program performance.
