- Assessing client needs and developing individualized care plans
- Coordinating and monitoring services provided to clients
- Collaborating with healthcare professionals and community resources to ensure comprehensive care
- Conducting regular client assessments and reassessments to evaluate progress and adjust care plans as needed
- Providing counseling and support to clients and their families
- Maintaining accurate and up-to-date client records
-assessing treatment needs developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches;
-Admits new clients by reviewing records and applications

- Proficient in Windows operating systems for software development and testing purposes
