Angel senior care is Hiring a Care Manager Near Spokane, WA


Responsibilities Client Care Management:
 Conduct intake assessment, needs assessment, treatment planning, reassessment of services and write the plan of care » Provide day-to day support, supervision, oversight and follow through for clients » Reviews client cases with the team and provides advice, direction, and support as needed » Screen clients for eligibility for direct and support services and refer clients to needed services, such as placement, mental health, housing, crisis, and employment assistance 
 Facilitate Care Team meetings to discuss client care plan and share information regarding client change in condition » Document client services in medical records though AxisCare and other computer programs » Establish and retain good relationships with clients, family members, vendors and the office personnel » Maintain documentation of all client encounters and complete reporting requirements according to organization standards » Track client information, schedules, files, and forms in a confidential manner » Handle scheduling conflicts and work with scheduling covering vacations and as necessary » Fill in and provide client care in an emergency
» Attend and represent the organization at professional conferences, in-service training, and meetings at the request of or with the approval of supervisor » Conduct quality assurance and monitoring activities for service delivery and documentation, work closely with sales and lead caregivers 
Commitment to the mission of care coordination » Good communication and interpersonal skills and ability to speak concisely to clients, families, caregivers, and interact with care team members » Organized with confidential client material, appointment tracking, and caseloads » Ability to build relationships with different types of people, including referral sources, community organizations, organization members, and members of external health care teams » Must be organized, have the ability to multi-task, spell and write using proper punctuation and grammar, ability to "drop and go" during a crisis and have empathy and compassion for seniors »
