CHCW has an opening for a Population Health Nurse to collaborate with our clinical teams, contracted payors, and patients to address gaps in care. You will be responsible to develop and monitor patient registries and patient education to improve adherence to evidence-based preventive medicine. This position offers the opportunity for a hybrid remote/onsite work atmosphere.

Qualified applicants will have a BSN with a minimum of 1 year clinical nursing experience or ASN with applicable experience will be considered. The ideal candidate will have prior experience in Population Health initiatives like chronic disease management, care management, or utilization management. Bilingual Spanish/English preferred and a plus! Pre-employment drug screen required.
