New Hope is a nurse-driven clinic at Miramar Health Center, a Yakima Valley Farm Workers Clinic. New Hope specializes in caring for patients living with HIV in Central Washington, offering medical, dental, nutrition, counseling, outreach, and mental health services. We are a leader in the Central WA region for HIV care. Public health nurses and case managers collaborate to provide patients with wrap-around care services. As a Public Health Nurse, you will have the opportunity to:

  • Provides comprehensive specialized care and support to patients living with HIV/AIDS by monitoring and assessing patients' health, administering medications, and managing symptoms related to HIV/AIDS.
  • Provides emotional support and counseling to patients, addressing their fears, concerns, and challenges related to living with HIV/AIDS.
  • Maintains wellness activities for patients, including immunizations, screening exams, and follow-up lab work. Offers guidance on safe practices, including safe sex and needle exchange programs comma, to prevent the spread of HIV/AIDS.
  • Develops care plans with patients in collaboration with providers based on medical conditions, health goals, and behaviors. Collaborates with other healthcare providers in developing a plan of care. Coordinates and plans interagency referrals for patient care as directed by the provider.
  • Provides assessment, education, and evaluation of patients' readiness to begin Antiretroviral Therapy (ART). Provides ongoing long-term management for patients receiving HAART therapy.
  • Administers medication to patients as directed by the provider, protocol, and standing orders. Identifies the purpose, side effects, and onset of action for each drug class given to the patient. Correctly explains medication to patient/guardian and obtains consent. Regularly monitors patients' viral loads, CD4 counts, and overall health to track progress and adjust treatment plans as necessary.
  • Follow strict infection control protocols to prevent the transmission of HIV/AIDs within the clinic setting.
  • Maintains appropriate ongoing assessment, interventions, education, and referrals as identified in the care plan.
  • Maintains regular contact and works collaboratively with other services and professionals involved in delivering services to clients to ensure coordinated service delivery.
  • Provides HIV counseling and coordination of testing for high-risk individuals.
  • Completes chart notes per standards in EPIC and Provides systems. Updates client data in the Electronic Medical Record (EMR) systems to document health assessment and health history. Records care plans and education provided. Documents assessments, actions, and medications in medical records.
  • Provides education to a wide variety of audiences, including community members and healthcare professionals, regarding HIV-including transmission and risk reduction. Community outreach programs to raise awareness about HIV/AIDS, promote testing, and provide support to affected individuals and their families.
  • Advocates for patients' rights and access to health care services, ensuring they receive appropriate medical and social support
  • Crisis management: effectively handles medical emergencies and crises, providing immediate care and coordinating with the health care team. This includes taking calls for after hours.
  • Continuing education: stay updated with the latest advancements in HIV/AIDS treatments, medications, and healthcare practices through continued education and training
  • Performs other duties as assigned.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), Department of Health (DOH), and YVFWC requirements.

Qualifications:

  • Education: An Associate's Degree in Nursing required. Bachelor's Degree in Nursing preferred.
  • Professional Licenses/Certificates/Registration: WA state RN license. CPR certification within 90 days of employment. BLS Certification is required. ACLS Certification preferred. Valid Driver's License and proof of automobile liability insurance coverage.
  • Knowledge/Skills/Abilities: Bilingual (English/Spanish) welcomed; Bilingual differential pay requires level 10 on the language proficiency scale. Knowledge of established professional nursing concepts, procedures, and medication uses and effects. Effective verbal, written, and listening communication skills. Ability to prioritize work and handle a variety of tasks with frequent interruptions. Skills to effectively interact professionally with patients, families, and employees. Skills to understand and meet the unique needs of patients of all ages. Basic proficiency in Word, Excel, and EMR.
