Registered Dietician

$25-$27 per visit

$18-20 in office

Duties of the Position

• Direct, supervise, evaluate, and provide nutritional services on an intermittent basis to patients in their homes in accordance with the physician’s orders.

Responsibilities

*Assess and evaluate the nutritional status of patients. *Assist the physician and nursing personnel in determining the dietary needs of the patient. *Prepare and calculate therapeutic diets to meet the specific requirements of the patient and confer with the physician to establish dietary treatment. *Assist the patient and family to understand and follow dietary modifications ordered by the physician. Instructs on dietary restrictions and drug/nutrient interactions as appropriate. *Observe and report to the physician and nurse supervisor the patient’s response to dietary treatment and any related changes in the patient’s condition and revise the nutritional plan as indicated by the patient’s condition. *Participate in care coordination activities, and act as a resource to other healthcare staff, as requested. *Participate in Quality Improvement activities as assigned and communicate information that enables the collection and analysis of data to identify opportunities for improvement. *Participate in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the patient's status and obtaining additional orders if necessary. *Communicate with other team members and, when appropriate, instruct them in nutritional, and dietary teaching that they may use while working with the patient. *Maintain and submit written clinical and agency records as deemed necessary. *Participate with staff, patients, and physicians in discharge activities and complete a Dietary Discharge Summary. *Assume responsibility for self-development by continually striving to improve his/her healthcare knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings, and individual research and reading. *Confirm weekly the scheduling of visits with the Clinical Manager to coordinate Agency visits. *Perform additional duties assigned in response to patient and/or Agency needs.

Assists in the Admission Process

*Obtain a medical history, particularly as it relates to the present condition. *Evaluate the patient, the patient’s significant other(s), and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the patient, and what other Agency and community services will be required. *Assure that the physician’s orders are appropriate and discusses necessary changes.

Job Conditions

*The ability to drive and the ability to access patients' homes which may not be routinely wheelchair accessible is required. *Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care. *On occasion, may be required to bend, stoop, reach, and move a patient weighing up to 250 pounds, and/or carry up to 30 pounds. *Must be able to effectively communicate in English.

Equipment Operation

• Use of nutritional teaching literature and hand washing materials.

Qualifications

*Must be a college graduate. *Licensed or registered by the State. *Must have at least one year of experience. *Must have a Criminal Background Check. *Must have a current CPR certification.

Available shifts and compensation: We have available shifts all days of the week. Compensation is $25.00 - $27.00/hour.
