Only Angels Home Healthcare LLC located at 1624 S. Rutan Ave is currently looking to add a Social Worker to our growing team! We take pride in supporting our local community through-out the Hospice experience and provide additional resources to our Home Health patients.
Responsibilities:
- Conduct diagnostic evaluations and assessments to determine the needs of clients
- Develop and implement treatment plans based on client needs and goals
- Collaborate with other healthcare professionals to coordinate care for clients
- Advocate for clients' rights and access to resources
- Provide crisis intervention and support during emergencies
- Maintain accurate and confidential client records
Requirements:
- Bachelor's or Master's degree in Social Work or related field
- Valid licensure/certification as a Social Worker (LCSW, LMSW, etc.)
- Knowledge of applied behavior analysis principles and techniques
- Familiarity with HIPAA regulations and guidelines for maintaining client confidentiality
- Experience in dementia care, nursing home settings, or family counseling preferred
- Strong coding and research skills for documentation purposes
- Ability to conduct diagnostic evaluations and provide appropriate treatment recommendations
- Knowledge of various psychotherapy modalities and interventions
- Experience working with individuals with PTSD or trauma-related issues is a plus
