Job Summary : Responsible for entering all stroke program patients' information into respective state and national registries in a timely and accurate manner.
Responsible for all related clerical reports, data, forms, and registry. Assists with research documentation as needed.
Licensure, Registration and / or Certification : None.
Work Experience : 1-2 years minimum in Healthcare related experience, or related office experience
Knowledge, Skills and Abilities : Effective verbal and written communication skills, tact and positive demeanor. Effective computer skills, (i.
e., Excel, Word, Power Point, etc.) Ability and desire to establish and maintain rapport with physicians and organizational personnel.
Essential Functions and Responsibilities : Performs daily abstraction from the medical record, ambulance forms and logs, including all data elements as required by the AHA / ASA - GWTG standards, as well as DNV standards.
Enters data into GWTG and State Stroke Program registry, checks for accuracy and completion of data. Provides accurate and complete routine and ad hoc reports in a timely manner.
Assists with preparation of data and quality metrics for Stroke Performance Improvement Committee, Stroke Workgroup Committee, SA RN Meeting, and Neuro Council Meeting.
Assists with case-finding / identification of cases seen or treated at Saint Francis Hospital for research projects identified by the Stroke Program and required documentation as needed.
Decision Making : Independent judgment in making minor decisions where alternatives are limited and standard policies / protocols have been established
Working Relationships : Works with internal customers via telephone or face to face interaction Works with external customers via telephone or face to face interaction Works with other healthcare professionals and staff
