Duties:
Aall Care Home Health is seeking for immediate hire of an Advantage Case Manager to join the Aall Care Team! Advantage Case Managers educate, analyze plans, coordinate service delivery, evaluate member's outcomes resulting from the care received. The case manager create service plan/goals and acts as a liaison between members/staff/community.

Must have a current Advantage Case Management Certification, Harmony experience, strong documentation skills, and the ability to work autonomously. Full-time, no weekend or after hours. Valid driver's license and a baccalaureate degree in a human service field or nursing degree Rn/Lpn and able to manage a caseload of individuals in Tulsa and surrounding areas.
