LPN Case Manager
Overview:
We are seeking a dedicated LPN Case Manager to oversee the comprehensive case management process for our patients. The role involves assessment, service planning, and ongoing monitoring to ensure optimal patient care and outcomes.
Key Responsibilities:
• Identify and assess high-risk patients, conducting initial screenings for eligibility determination.
• Develop and implement comprehensive treatment plans tailored to individual patient needs.
• Collaborate closely with the care team to ensure coordinated delivery of services.
• Engage with patients to facilitate understanding of goals and support them in achieving and monitoring progress.
• Lead Transition of Care (TOC) activities to ensure seamless transitions between care settings.
• Refer patients with identified social determinants of health to Community Health Worker or appropriate designee.
• Document all services provided and coordinate effectively with the interdisciplinary team.
Qualifications:
• Current Oklahoma nursing (LPN) license in good standing.
• Minimum of 2 years of clinical experience in healthcare.
• At least 1 year of experience in Clinical Case Management or Care Management.
• Current BLS/CPR certification.
• Demonstrated ability to work effectively with staff and the public.

Working Conditions:
• Work primarily takes place in a clinic environment.
• Ability to lift a minimum of 15 pounds, occasionally up to 25 pounds.
• Ability to sit and stand for extended periods, with bending, lifting, and occasional stair climbing required.
• Some exposure to operating circumstances requiring adherence to standard safety precautions may occur.
