North Olympic Healthcare Network is Hiring a Care Management Coordinator Near Port Angeles, WA

Position SummaryThe Care Management Program Coordinator will work together with other members of the North Olympic Health Network Care Team to provide high quality, well-coordinated, highly organized, patient-centered care to patients and their families/care givers/support systems. The Care Management Program Coordinator will work in several capacities as a support role to the Provider-led Care Team.
Primary AccountabilitiesAt North Olympic Health Network, the Care Management Coordinator is a specialized role providing support to care teams and the care management programs with specific focus on OBAT Program support, Clallam Care Connections (3C) Program support, and other Care Management Programs support as needed.
  • Care Coordination program support including backline coverage as needed.
  • Coordinates and leads Care Management meetings as needed (including note taking and preparing agendas)
  • Care Managment Programs billing support and documentation
  • Care Management Programs database management and patient tracking.
  • Creating and managing reports relevant to Care Management Programs.
  • Serves as a core member of the OBAT care team and has the following specific accountabilities:
  • Acts as a non-judgmental, knowledgeable, accessible liaison for patients and outside agencies interacting with the OBAT program and other departments within NOHN
  • Help assure compliance with Federal, State and program guidelines and requirements pertaining to the care of patients with opioid use disorder. Safeguard confidential information.
  • Works with the Behavioral Health Manager, OBAT clinical nurses, and office support staff to implement best practices for office-based opioid treatment and care coordination.
  • Provides services including OBAT patient screenings, scheduling appointments and assisting patients with insurance coverage issues when needed.
  • Attends and actively participates in OBAT collaborative meetings, team meetings, and trainings as assigned.
  • Provides direct non-clinical program support and administrative services for the BAT clinical nurses and OBAT providers.
  • Communicates and collaborates with providers and all members of the multi-disciplinary health care team as needed to facilitate OBAT patient registries, care delivery appointments, and community outreach activities.
  • Works in partnership with external addiction facilities, mental health, and primary care agencies toward goals of collaborated care for patients, including routine communication with SUD treatment centers, and helping patients gain admission into treatment centers, as appropriate.
  • Conducts appropriate community outreach with prospective referral sources.
  • Completes all required OBAT and new patient paperwork with new patients.
  • Assures the OBAT program follows NOHN administrative protocols.
  • Assists with OBAT grant tracking when applicable
  • Work in cooperation with the OBAT clinical nurses and other NOHN staff to assist patients through the healthcare system by acting as a patient advocate.

  • Works collaboratively with all staff, providers and leadership
  • Engages others as part of a team-oriented philosophy
  • The Care Coordinator will work with practice leadership, providers, clinical staff, and ancillary care teams, as well as with patients, families/caregivers, to achieve healthcare and lifestyle goals and maintain open lines of communication across the care team.
Position RequirementsEducation
  • Care Coordinators assisting medical teams must be proficient in medical terminology, basic anatomy & physiology
  • Care Coordinators assisting OBAT team must have any equivalent combination of knowledge, skills, abilities, education, and experience:
  • Education: Bachelor's degree in social science or health related field preferred.
  • Experience: A minimum of two (2) years of program coordination/facilitation in a healthcare setting is preferred. Experience with trauma informed care and anti-stigma work preferred.
  • Licensure/Certification: Washington State Chemical Dependency Professional or Professional Trainee license are a plus but not required.

  • Demonstrate patient-centric, customer service-oriented approach

Physical/Environmental
  • Must be able to walk intermittently for 8 hours.
  • Some standing, sitting, bending, occasional lifting.
  • Use hands and fingers to continually manipulate a keyboard.
  • Must be able to verbally communicate and hear.
  • Viewing the computer for 8 hours.
