Crisis Care CoordinatorHiring Range: $24.94- $30.55 perhour

General Summary of Duties: The Care Coordinator functions as a member of a multidisciplinary team while demonstrating the clinical skill and experience to plan, develop, coordinate, and provide treatment, and support services to program clients. A Care Coordinator also provides education and crisis intervention services to clients in crisis services. The care coordinator also works with the team throughout the client’s stay in crisis services to ensure they receive the appropriate resources and referrals upon discharge.  

Major Responsibilities/Tasks:  

  • Provide care coordination services for clients who are currently in crisis services. Provide direct services such as individual, group, crisis intervention, and case management to clients in crisis services.
  • Advocate for clients’ rights and preferences. Consult with community agencies such as DSHS and court systemsto maintain coordination in the treatment process.
  • Collaborate with the case manager to assist and support clients in finding and effectively using a primary care physician and other healthcare services. Provide education to clients to enable them to develop health self-management skills. Where appropriate work with the supervisor to coordinate psychiatric care for the client.
  • Provide crisis intervention counseling and coordinate with Designated Crisis Responders/crisis services when appropriate, in collaboration with the mental health professional.
  • Document client progress to maintain a permanent record of client activity according to established policies and procedures.
  • Participate in daily staff organizational meetings and treatment planning review meetings.
  • Participate in providing information to clients on substance use issues andcoordinate treatment with internal and external substance use treatment providers.
  • Assist clients with gaining resources and education to support their autonomy and basic needs.
  • In collaboration with the mental health professionaland client develop, implement, and revise overall treatment goals and plans.
  • Coordinate with treatment team to facilitate timely implementation of discharge plans for clients being served on the crisis unit. Develop and coordinate discharge plans that are specific to clients.
  • Maintains the clinical information of individuals served by the unit in a confidential manner.

Minimum Qualifications:  

EDUCATION: Bachelor’s Degree in Psychology, social services, or behavioral health field. 

EXPERIENCE:minimum 2 years job-related experience 

This position requires driving clients in a personal vehicle on behalf of the agency; therefore, the incumbent must successfully complete a motor vehicle history check, possess and maintain a current, valid driver's license in the state of Washington, and have reliable, insured transportation. 

LICENSURE: Agency Affiliated Counselor Registration.  

Preferred Qualifications: 

EXPERIENCE:Minimum 5years of job-related experience 

Experience in co-occurring disorders treatment, crisis services or inpatient psychiatric services.
