Health Right 360 is Hiring an Access Specialist Near Pomona, CA

HealthRIGHT 360 gives hope, builds health, and changes lives for people in need by providing comprehensive, integrated, compassionate care that includes primary medical care, mental health services, and substance use disorder treatment. Our Outpatient services specialize in treatment mental health and substance use disorders with treatment and skills groups, as well as community resources.
Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client’s specific needs and goals. Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Job Overview
This is a union position.
Prototypes Outpatient (Pomona) is a Certified Community Behavioral Health Clinic (CCBHC), a grant funded program. The CCBHC model of care focuses on a client's whole person health, incorporating behavioral health and physical health care needs. The CCBHC program allows us to enhance the established behavioral health services our agency has been providing for decades and connect clients with community partners through care coordination partnerships in the area.
This is a grant funded position through SAMHSA contract (H79SM086855) funded through 09/30/2026.
The (CCBHC) ACCESS Specialist is a registered substance use disorder professional who maintains registration from an approved/accredited California agency. The (CCBHC) ACCESS Specialist facilitates referrals and access to treatment for the community. The (CCBHC) ACCESS Specialist offers screening, referrals, and follow-up services to clients to increase self-efficacy, self-advocacy, basic life skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and reintegration into the community. The (CCBHC) ACCESS Specialist is responsible for coordinating, tracking, and maintaining status updates reflective of time to services in the EHR system for data required to be reported by all CCBHCs.
Key Responsibilities
  • Respond to all phone, web, and walk in inquiries about all programs offered across campus, as well as programs offered at other locations.
  • Collaborate with community agencies to ensure access to treatment.
  • Complete brief screenings to ensure candidate is referred to appropriate care.
  • Track and document pre- admissions paperwork, provide information for intake appointments, create client profiles, and input information into EHR.
  • Keep time to service data in EHR up-to-date to ensure accuracy of this quality measure information for CCBHC reporting requirements.
Education and Knowledge, Skills and Abilities
Education Requirements:
  • High School diploma or equivalent.
  • Registered as an Alcohol and Drug Counselor with an approved state agency and have completed a minimum of 90 in-class hours toward certification.
Experience:
  • Preferably a minimum of two years’ experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.
  • Experience working with clients experiencing acute withdrawal from substances.
  • Knowledge of and experience with providing trauma informed services and managing crisis.
  • Experience delivering evidence-based practices.
Background Clearance Required:
  • CDCR fingerprint clearance.
  • Must not be on parole or probation for a minimum of two years.
Knowledge Required:
  • Knowledge issues of related to substance abuse, mental health, and criminal background.
  • Culturally competent and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and internet applications.
  • Bilingual- some positions may require a second language that reflects the client population.

Haight Ashbury Free Clinics and Walden House have both grown over the years, becoming national models for community healthcare, substance use disorder treatment and mental health services. In 2014, North County Serenity House of San Diego county and Womens Recovery Association (WRA) of San Mateo joined HealthRIGHT 360, continuing its leadership as a provider of gender responsive services for women and women with children. In 2015 Lyon-Martin Health Services became the third primary care clinic operated by HealthRIGHT 360, continuing its specialized care for women and transgender individuals. Tenderloin Health Services (formerly Glide Health Services clinic) became the fourth primary care clinic in 2015, providing care in San Franciscos Tenderloin neighborhood. Prototypes joined in 2016 expanding behavioral health care for women and children and services to survivors of domestic violence in Los Angeles, Orange and Ventura Counties. Strengthening services for women and girls in San Francisco, Womens Community Clinic joined our family of programs in 2017.
