Job Overview
This is a union position.
The mission of healthRIGHT360 is to give people in need a chance for hope, change, and the tools to build a better life. We do this by providing compassionate integrated care that includes primary medical, mental health and substance use disorder treatment.
The Community Outreach Worker works in collaboration and continuous partnership with the healthcare teams to provide community outreach to link individuals to HealthRIGHT 360 for integrated primary care, behavioral health, substance use disorder treatment, oral healthcare, enabling resources and other services provided within the agency. This may include both in reach to other HealthRIGHT 360 programs as well outreach to single room occupancy hotels, navigation centers, community resources such as free dining rooms, and other areas that the target population may congregate. This position require flexibility in hours that may include working evenings and weekends.
Key Responsibilities
Outreach responsibilities: Provides internal program and direct community outreach to targeted populations; Participates in health fairs, neighborhood community activities, screenings and special events; Provides outreach to Single Room Occupancy hotels, street outreach, and goes to areas where target populations may congregate such as navigation centers. Creates liaison with other social service organizations for referral of patients. Participates in internal behavioral health meetings to engage patients in primary care; Refers patients to the medical clinic for primary medical care; Refers patients to Substance Use Disorder treatment programs for detox, residential, and outpatient services; Assist in the planning and coordination of community health fairs and events; May assist in the development and implementation of a marketing campaign to the community served by HealthRIGHT 360; Maintains comprehensive documentation for data collection; and provide statistical data and/or reports as requested or required. Engages newly assigned patients from the health plan, as well as those that have lapsed in care. Outreaches to established patients to engage in clinical quality measures and panel management for preventative health and chronic disease management. Outreach may include evening and weekend activities.
General Duties and Responsibilities: Demonstrates a clear understanding of HealthRIGHT 360’s Mission, Purpose and Philosophy; Adheres to ethical business practices by striving to perform in a manner that conforms to the highest standards of ethical behavior, integrity, and honesty; Takes initiative and responsibility for decisions as an individual. Exhibits commitment to personal and company wide goals; Participates in the ongoing development of competencies, capabilities, technology, and the resources needed to achieve high standards of efficiency and effectiveness. Approaches job duties and responsibilities with the passion and a desire to meet highest standards; Take a patient centered approach; Works well with healthcare team to bring patients in to care; Encourages a positive environment and experience for coworkers and patients; Treats patient and coworkers with dignity and respect; Maintains confidentiality of all agency and patient related information; Assists with special projects as directed by the Admissions Director in collaboration with leadership and Executive teams.
Documentation responsibilities: Ensures proper tracking of outreach efforts through thorough documentation; this documentation will include but is not limited to documentation in the electronic medical record system. Additionally, must document contacts on a dashboard that is reported on a regular basis to the Outreach Manager.

Education and Knowledge, Skills and Abilities
Education Required:
  • GED, high school diploma or higher degree from accredited University/College.
  • Written and verbal English literacy and ability to explain healthcare concepts.
  • Excellent communication and interpersonal skills and problem-solving abilities.
  • Ability to work cooperatively with others and demonstrate good judgment in unusual and emergent situations.
  • Ability to work independently and manage multiple tasks simultaneously.
  • Computer literacy with Microsoft Office products, Microsoft Outlook.
Preferred:
  • Bi-lingual in Spanish.
Experience:
  • Experience working with populations from various cultures and backgrounds and demonstrates ability to provide culturally competent support to patients.
  • Experience working with homeless, mentally ill, substance using, and criminal justice patients.
  • Experience working with populations with varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds, and economic status.
  • Experience working with populations suffering from addiction.

History To address the thousands of adolescents and young adults that were streaming into San Francisco for the cultural revolution of the 1960s, Haight Ashbury Free Clinics (HAFC) opened its doors in 1967 as the first free medical clinic in the country. During the first week of operation over 400 patients were seen. HAFC has been an innovator in d...elivering primary health care services to many of the people who can least afford them. Health Care is a Right, Not a Privilege has been the guiding principle as well as its famous tagline. Walden House was founded in 1969 in the same Haight-Ashbury district of San Francisco to help homeless and runaway adolescents with substance use disorder problems. Today, Walden House treats people with mental health and substance use disorder problems at various residential and outpatient centers throughout California, including in-prison treatment programs, and facilities in San Francisco and Los Angeles, providing drug and alcohol treatment and mental health, vocational and housing services for people transitioning back into their communities. Like HAFC, Walden House has always served people who are uninsured, homeless and socio-economically disenfranchised, including those with HIV/AIDS. Haight Ashbury Free Clinics and Walden House have both grown over the years, becoming national models for community healthcare, substance use disorder treatment and mental health services. The organizations merged on July 1, 2011 to best serve the most vulnerable members of our community. On July 1, 2012, Haight Ashbury Free Clinics Walden House adopted a new name: HealthRIGHT 360. A Growing Family of Programs Asian American Recovery Services (AARS) joined the family of HealthRIGHT 360 programs in 2013. Founded in 1985, AARS has grown to serve thousands of people throughout San Francisco, San Mateo, and Santa Clara counties. In 2014, North County Serenity House of San Diego county and Womens Recovery Association (WRA) of San Mateo joined HealthRIGHT 360, continuing its leadership as a provider of gender responsive services for women and women with children. In 2015 Lyon-Martin Health Services became the third primary care clinic operated by HealthRIGHT 360, continuing its specialized care for women and transgender individuals. Tenderloin Health Services (formerly Glide Health Services clinic) became the fourth primary care clinic in 2015, providing care in San Franciscos Tenderloin neighborhood. Prototypes joined in 2016 expanding behavioral health care for women and children and services to survivors of domestic violence in Los Angeles, Orange and Ventura Counties. Strengthening services for women and girls in San Francisco, Womens Community Clinic joined our family of programs in 2017.
