Boston Public Health Commission is Hiring an Admissions Coordinator Near Mattapan, MA

The mission of the Boston Public Health Commission (BPHC) is to protect, preserve, and promote the health and well-being of all Boston residents, particularly the most vulnerable. The BPHC envisions a thriving Boston where all residents live healthy, fulfilling lives free of racism, poverty, violence, and other systems of oppression. The BPHC sets an expectation that all staff and leadership commit, individually and as part of the BPHC team, to hold ourselves accountable to establishing a culture of antiracism and advance racial equity and justice through each of our bureaus, programs, and offices.

DUTIES

  • Oversees and coordinates all referrals, screenings, admissions, intakes, discharges, tracking, and pre-billing of clients of Homeless Services' Transitions program.
  • Responsible for reviewing and completing MIS data and providing data to BSAS, BPHC, Homeless Services and other agencies as required.
  • Responsible for confirmation of all payers/funding sources (BSAS, MBHP, NHP, etc.) authorization of client participation and length of stay at Transitions.
  • Responsible for maintenance, accuracy and transmission of all admission, discharge and pre-billing data, including IVR logs, census, admission and discharge logs, program logs, spreadsheets, and all other required materials.
  • Provides supervision to assigned staff including hiring, scheduling, training and evaluation.
  • Collaborates with Program Administrator to develop and implement admissions and discharge policies and procedures.
  • Oversees appropriate filing of admission/discharge materials in client charts, appropriate and timely filing of client charts in chart room, maintenance of chart room in organized manner, and oversees disposal of charts as required by the BPHC/BSAS guidelines.
  • Collates and evaluates data pertaining to admission/discharge/tracking/billing and all required statistics, and ensures that all relevant data is submitted in accordance with guidelines.
  • Coordinates various client assessments, surveys and interviews.
  • Fulfills administrative support responsibilities per BSAS Transitional Support Services contract.
  • Oversees the tracking of clients after discharge. Collates and evaluates data as required.
  • Develops positive professional relationships with relevant agencies to facilitate access to Transitions.
  • Working to help BPHC become an antiracist organization by meeting or exceeding the standards set by the BPHC Anti-Racism Policy.
  • Attends meetings as required.
  • Performs other duties as required.
