Harbor Health also operates two Programs for All-Inclusive Care for the Elderly in Mattapan and Brockton, providing comprehensive healthcare, transportation and social services for more than 580 frail elders who continue to live with dignity and independence in the community and a Woman, Infants and Children (WIC) Nutrition Program. To help further the mission of Harbor, we are looking for a talented Director of Nursing to join our Leadership team at our Community Health Centers in Boston, South Shore and Cape Cod.

Role: 

The Director of Nursing (DON) leads the nursing and clinical support functions of Harbor Health Community Health Centers to include the Daniel Driscoll Neponset Health Center, Geiger Gibson Community Health Center both located in Dorchester, Harbor Community Health Center-Plymouth and Harbor Community Health Center-Hyannis. The DON is responsible for recruiting, developing and retaining a high performing team of Nurses and Medical Assistants. The DON leads the design and implementation of clinical workflows that deliver safe, high quality, patient-centered care, in collaboration with physician and administrative leadership.

Responsibilities: 

Leadership and Administration

  • Provides visionary leadership to the nurses and medical assistants, fostering a positive and collaborative work environment.
  • Promotes and champions Harbor Health core values of impact, inclusion and innovation.
  • Supports and supervises nurse leaders at each health center site.
  • Develops annual staffing model and budgets for responsible areas.

 Care Transformation:

  • Oversees implementation of complex care support models and collaboration with external ACO partner.
  • Leads innovation and development of nursing and Medical Assistant (MA) roles to support transition from fee for service to capitated payment models.
  • Maximizes the contributions of Nurse and MA staff through standing orders and other opportunities for all staff to work to ‘top of license’. 

 Quality Improvement:

  • Lead initiatives to improve the quality of patient care and clinical outcomes, with particular focus on health equity and disparities between populations.
  • Monitor and evaluate nurse and MA performance metrics, identifying areas for improvement and implementing evidence-based practices.
  • Develops and maintains policies and guidelines related to nursing and medical assistant responsibilities.

  Staff Development: 

  • Recruits, hires, and trains Nurse and MA staff, ensuring compliance with licensure and certification requirements and annual competency verifications.
  • Provides ongoing professional development opportunities for Nurse and MA staff to enhance their skills and knowledge.

Patient-Centered Care:

  • Promotes and maintains a culture of patient-centered care, emphasizing compassion, dignity, and respect in all interactions.
  • Collaborates with other healthcare providers to ensure seamless and coordinated patient care.
  • Identifies and implements opportunities to improve patient experience, with particular focus on equity and inclusion of diverse patient experiences and perspectives.

 Compliance/Employee Health/Infection Control:

  • Maintains up-to-date knowledge of federal, state and local laws and regulations related to the delivery of care i.e. requirements set forth by the Board of Registration in Nursing, Department of Public Health and the Centers for Disease Control.
  • Ensures Harbor’s continued certification as an NCQA Patient Centered Medical Home.
  • Ensures timely reporting and response to all patient care incidents, and contributes to Harbor Health’s risk management efforts.
  • Supports employee health vaccination efforts by coordinating site-based RN vaccination administration.
  • Ensures adherence to all infection control practices, including proper sterilization, cleaning and maintenance of spaces and equipment.
  • Coordinates with the Compliance Officer for notification of local and state health departments of any reportable diseases.
  • Oversees compliance and staff competency activities related to CLIA-waived lab testing.

 Community Engagement/External Affairs:

  • Fosters strong relationships with community partners, delivering community-based clinics and health and wellness initiatives.
  • Represents the community health center in relevant community forums and events.
  • Develops relationships with educational institutions to create opportunities for Harbor Health staff development, as well as practical training opportunities/pipelines for future staff.
  • Represents Harbor Health in external associations and forums such as Massachusetts League of Community Health Centers and the National Association of Community Health Centers.

Requirements:

  • RN required. BSN or Masters Degree (MSN) preferred
  • 7 or more years of nursing experience with at least five (5) years of experience managing nurses
  • Federally Qualified Health Center (FQHC) or Ambulatory nursing experience preferred
  • Intermediate knowledge of Office 365 including Outlook, Word, Excel and Microsoft Teams
  • Experience using Electronic Medical Record (EMR), EPIC/OCHIN preferred
  • Familiarity with HRSA, DPH and ACO regulations preferred
  • Must be able to travel between Harbor sites in a timely manner 
  • Must be able to attend Board and community evening meetings when necessary with occasional travel to national conferences and workshops as needed

Supervisory Responsibilities:

  • Supervises Clinical Nurse Managers
