Job Summary:

  • Updating patient specialty care plans
  • Preparing patients and providers for upcoming visits
  • Coordinating care with community partners
  • Coordinating post-hospital plans of care
  • Collaborating with the Interdisciplinary Care Team (IDC)

Nurse Care Manager Knowledge/Skills/Abilities:

  • Strong computer skills
  • Ability to multi-task at a very high level
  • Strong problem-solving abilities
  • Excellent phone/customer service skills
  • Familiarity with EMR software 
  • Strong Clinical Background
  • Ability and willingness to expand clinical knowledge

Required:

  • Current Registered Nurse (RN)
  • Minimum 3 years of clinical experience

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $39.00 - $45.00 hourly with the potential for performance-based bonuses and other benefits.
