Patient Care Coordinator
Community Health Services is seeking a Patient Care Coordinator to join our team! This is a full-time position, no evenings, weekends or holidays! CHS employs those who are eager to grow professionally, gain great experience and work with a terrific team. The Patient Care Coordinator will be responsible for managing referrals, reviewing patient charts, assisting with insurance enrollment, and maintaining communication with providers to provide comprehensive patient care.
Hours for this position are: Monday 7a-7p, Tuesday- Thursday 8a-5p, & Friday 8a-1p
Qualified candidates must have the following to be considered for employment:
- Associate’s Degree in social work or equivalent health-related field required
- Previous experience in healthcare coordination or related field preferred
- Excellent communication skills, both written and verbal, with the ability to communicate effectively with patients, healthcare providers, and external agencies
- Highly organized with strong attention to detail, capable of managing multiple tasks and priorities efficiently
- Compassionate and empathetic demeanor, with a genuine commitment to patient care and advocacy
- Able to read and write in English
