We are seeking a skilled and compassionate Nurse Practitioner to join our healthcare team working in a Nursing Home setting. You will be assigned a daily patient caseload of 20-25 patients, and will conduct assessments, diagnosis, treatments and ordering labs etc. as needed. You will collaborate with physicians and other healthcare professionals to coordinate patient care, as well as with the patients families to educate and discuss the patients care.

Experience:
- Preferably of 2 years of experience as a Nurse Practitioner in a clinical setting

- Proficiency in medical documentation using electronic health record systems
- Strong knowledge and experience in patient assessment, diagnosis, and treatment planning

MUST HAVE AN ACTIVE NY LICENSE AND BE BOARD CERTIFIED
