Hunterdon Health is Hiring a Hospice Liaison Near Flemington, NJ

    • Group C if hired 12/31/15 or earlier

Position Summary

Responsible for developing, maintaining, and growing relationships and increasing referrals to Hunterdon Hospice and Palliative Care; referral sources include Hunterdon Medical Center and other hospitals, physicians and advanced practice professionals in primary care and specialty practices, nursing homes, assisted living communities, and other prospects.

The hospice liaison is responsible for increasing volume in hospice and palliative care and achieving sales goals as assigned.

Primary Position Responsibilities

  1. Maintains a core knowledge of clinical services provided by Hunterdon Hospice and Palliative Care; communicates those services to actual and potential referral sources using a variety of methods, including but not limited to in person visits and electronic tools, and follows up on communication to assess effectiveness. 
  2. Develops and maintains a close working relationship with all internal team members and disciplines; supports the care continuum by handing off patient, family and provider referral information to appropriate team members accurately and completely, including patient demographic data, insurance information, and primary care/referring physician. 
  3. Assists in planning and executing strategic marketing plans, including events, networking activities, social media campaigns, and promotional programs. 
  4. Is an active, collaborative member of the Hospice and Palliative Care interdisciplinary teams; Identifies opportunities to enhance customer service and patient/family satisfaction; Collects and uses data to determine marketing strategies and assess return on investment of system resources to support Hospice and Palliative Care.

Qualifications

  • Minimum Education:

Required: High School Diploma or GED

Preferred: Bachelor’s degree in business, Marketing or Health Sciences related field preferred

  • Minimum Years of Experience (Amount, Type and Variation):
    • Required:
      • Two years of experience in sales, marketing and/or healthcare industry
    • Preferred:
      • Hospice and Palliative Care Experience
  •  Knowledge, Skills and/or Abilities:
    • Required:
      • Familiar with general healthcare concepts and terminology; exceptional interpersonal and customer service skills, and a positive, enthusiastic attitude;; strong ability to problem solve. Must have strong written and verbal communication skills. Must be self-motivated and able to work independently, but have a strong commitment to teamwork. Must have excellent organization skills and commitment to quality. Must have excellent computer skills, including working knowledge of Microsoft and Google suite of products.

Preferred:

Knowledge of healthcare EMRs, including Netsmart, NextGen, QCPR and Epic; knowledge of hospice conditions of participation and federal regulation
