Description
Provides essential support to individuals seeking substance abuse and/or mental health services. The Case Manager will play a crucial role in coordinating and facilitating care for clients, ensuring they receive comprehensive and personalized treatment plans. This position requires strong communication skills, empathy, and a deep understanding of substance abuse and/or mental health principles.
Essential Functions:
· Perform client screening, intake, orientation, and assessment.
· Co-construct with clients an individualized treatment plan based on assessment and other collateral data; make referrals for services and resources as identified in plans; schedule appointments, provide or arrange for client transportation; and follow up on referrals.
· Provide individual case management sessions based upon the level of care requirements and complete a case management progress note.
· Document all client, family, collateral contact (f2f, phone, email, fax) in the clients' electronic health record.
· Support clients in developing motivation for recovery and engaging in recovery activities.
· Conduct psychoeducational and motivation enhancement groups.
· Develop and facilitate appropriate discharge planning with clients, in collaboration with the clients' family and treatment providers.
· Facilitate referrals to appropriate resources for immediate needs and aftercare services, such as, mental health providers, medication management, safe and sober housing, and other recovery supports.
· Maintain a working relationship with other agencies, community resources, and families to coordinate services for aftercare.
· Attend and actively participate in all staff meetings, treatment teams, supervision, and required training. Proactively coordinate with all facility departments to ensure the quality and continuity of care in accordance with clinical policy. Other duties as assigned.
Requirements
Minimum Qualifications:
· Bachelor's degree in Social Work, Psychology, Counseling, or a related field preferred.
· Proven experience in case management within a mental health treatment center or similar setting preferred.
· Strong knowledge of mental health diagnoses, treatment modalities, and recovery principles preferred.
