United Methodist Communities in Camden County, a 5-Star full-service senior living community that is a model in person-centered care community, located in beautiful Collingswood, NJ is seeking a Nurse Care Coordinator.


Requirements for a Nurse Care Coordinator:

  • Current NJ Licensed Practical Nurse (LPN) OR a Bachelor's degree in a related field from an accredited college or University.
  • Current NJ Registered Nurse License preferred.
  • One year's experience in case management marketing; Two years in case management marketing preferred.
  • Knowledge of Microsoft Office.

Responsibilities for a Nurse Care Coordinator:

  • Gathers and evaluates all referral information and reviews information received and/or assesses potential residents for admission.
  • Makes admission decisions and/or works with nursing on admission decision for healthcare.
  • Builds and maintains referral network, conducting onsite visits as needed.
  • Facilitate the admission process into healthcare and ensure paperwork is signed and processed in a timely manner.
  • Verify Medicare coverage and/or other benefits.
  • Participates in care conferences and other meetings as required.
  • Follows residents to home post discharge to ensure they are okay; completes medication reconciliation; psychosocial assessment.
  • Involved with post-acute discharge at home.
  • Ensures departmental reports are completed in a timely manner.
