North Central Missouri Mental Health Center is Hiring a Community Support Specialist Near Chillicothe, MO

This position is based in our Chillicothe office.

A. DEFINITION

The CPRC Community Support Specialist is responsible for providing ongoing community support and case management services to clients receiving services from the Community Psychiatric Rehabilitation Program.

B. DUTIES

  • Provide a minimum of 22 face-to-face client contacts per week.
  • Dictate or type documentation or related paperwork contemporaneously with service provision (within five (5) business days) and in accordance with agency and certification standards.
  • Provide ongoing assessment of client status including personal strengths, progress, problems and needs.
  • Provide preparation and/or revision of a specific individualized treatment plan per client.
  • Monitor client participation and progress in organized treatment programs to assure the planned provision of service according to each client's individual treatment plan.
  • Promotes measurable improvement in mental/medical health outcomes.
  • Communicate information about the client to all key staff who provide services to the client.
  • Complete required continuing education hours as required by certification standards.
  • May participate in 24-hour on-call rotation.
  • Intercede, with appropriate authorization, on behalf of individual clients within the community-at-large to assist the client in achieving and maintaining their community adjustment.
  • Maintain contact with clients who are hospitalized, and participate in and help facilitate discharge planning.
  • Provide individualized training in daily living skills (e.g. budgeting, housekeeping, cooking, personal grooming).
  • Provide individualized assistance in creating a personal support system that includes work with family members, legal guardians, or significant others regarding the needs and abilities of an identified client.
  • Assess and monitor clients’ adjustment to community living and provide guidance and support in the development of adaptive community living skills.
  • Cooperate with PSR and other agency group activities so as to incorporate the groups into the overall rehabilitative system of Community Psychiatric Rehabilitation programs.
  • Compile information regarding community resources.
  • Maintain client confidentiality.
  • Perform related work as requested to carry out the purposes, goals and objectives of the agency.

C. QUALIFICATIONS

  • A Qualified Mental Health Professional, or an individual with a bachelor’s degree in a human services field (i.e., education, nursing, criminal justice, recreational therapy, social work, psychology, human development and family studies, counseling, child development, gerontology, rehabilitation counseling, sociology); OR
  • An Associate of Applied Science in Behavioral Health Support from an approved institution; OR
  • At least two years of higher education with two years of experience in psychiatric, substance use treatment, or developmental disabilities; or any 4-year degree with two years of experience in psychiatric, substance use treatment, or developmental disabilities. Experience is to have included some combination of the following:
  • One-on-one, or group, services with a rehabilitation/habilitation and recovery/resiliency focus;
  • Taught and modeled for individuals how to cope and manage psychiatric, developmental, or substance use issues while encouraging the use of natural resources;
  • Supported efforts to find and maintain employment for individuals and/or to function appropriately in families, school and communities;
  • Assisted individuals with psychiatric, substance use or developmental disabilities to achieve the goals and objectives on their individualized or person-centered plan; OR
  • Four years of equivalent experience with consumers and their families receiving psychiatric, substance use, developmental disabilities services to move towards their personal, social and vocational competency in order to live successfully in the community. Experience to have included some combination of the following:
  • One-on-one, or group, services with a rehabilitation/habilitation and recovery/resiliency focus;
  • Taught and modeled for individuals how to cope and manage psychiatric, developmental, or substance use issues while encouraging the use of natural resources;
  • Supported efforts to find and maintain employment for individuals and/or to function appropriately in families, school and communities;
  • Assisted individuals with psychiatric, substance use or developmental disabilities to achieve the goals and objectives on their individualized or person-centered plan.
  • Knowledge of the community and services is preferred. Valid driver’s license, automobile and required liability insurance. Willingness to work flexible hours during the week and weekend. Integrity, honesty, openness, ability to cooperate, high level of initiative and self-confidence. Ability to relate to a variety of professionals and laypersons. Capacity to be empathetic and objective.
