As needed basis including PTO, leave coverage and weekend shortages.
Major Responsibilities :
Nursing / Therapy Coordination - Serve as internal point person and processing agent for all new home care referrals as well as serving as the Advocate representative responsible for coordinating ordered disciplines with staff therapists and contracted therapy agencies in the Home Care Region.
1)Coordinate and confirm receipt of new patient nursing and therapy referrals from CRD or other referral sources. Coordinate the processing of referrals and non-admits in STAT and other software applications where applicable.
2)Based on predetermined nursing care needs for new patients referrals, assign and schedule appropriately skilled field nursing staff to meet those care needs.
This process requires detailed communication with clinical personnel, clinical managers and with homecare patient and / or family.
3)Work with representatives of the contracted therapy vendors to coordinate and schedule therapy disciplines based on new patient referrals from referral sources.
This will require the coordinator to have detailed knowledge of coverage areas for staff therapists as well as each vendor and any referral volume guarantees and / or restrictions for each vendor.
4)Monitor weekly schedules for staff therapists ensuring that productivity volumes are maintained and work volume is manageable and evaluations and other paperwork are received in the office in a timely manner.
5)Coordinate and schedule nursing / therapy relief on an as needed basis while providing ongoing communication regarding new patients to appropriate North region non-clinical staff.
6)As needed, work with nursing staffing agencies as well as clinical coordinator and / or clinical manager to coordinate and schedule nursing and / or therapy visits by prioritizing those patients requiring priority care.
7)Maintain referral / scheduling log book to accurately reflect patient information, clinical staff assigned, ordered disciplines and date of receipt of start of care paperwork 8)Prepare weekend scheduling log of all Start of Care by contracted therapy vendor.
Coordinator must incorporate the priority patient scale of ortho, safety, PT opens and non-orthos for weekend scheduling in accordance with Standard Operating Processes.
9)Maintain clinical staff vacation and time off schedule for region as well as coordinate the preparation of the on-call therapy scheduling for contracted therapy vendors.
Communicate this schedule to clinical managers as needed.
Coordinate scheduling of home health aides to ensure appropriate and timely patient care 5)Work with clinical leadership in preparations of any reports required to monitor staff productivity and management of referral volume assigned to each staff member as well as agencies.
6)Insure compliance with HIPPA guidelines. 1)Advise home health aides of assignments. 2)Verifies daily home health aide’s assignments and maintains schedules.
3)Ensures home health aide coverage for any scheduled or non scheduled PTO too ensure appropriate and timely patient care 4)Contacts patients as needed to communicate schedule or staff changes as needed.
Performs other support responsibilities as requested. 1)Work with clinical leadership in preparations of any reports required to monitor staff productivity and management of referral volume assigned to each staff member as well as agencies.
2)Insure compliance with HIPPA guidelines.
Education / Experience Required :
High School graduate Second language a plus Knowledge of medical terminology 2-4 Years previous healthcare scheduling experience
Knowledge, Skills & Abilities Required :

Ability to coordinate and perform multiple tasks both effectively and efficiently Able to work under deadlines and time constraints Computer / data entry experience Working knowledge of Microsoft Word, Excel and Access as well as the ability to function in multiple mainframe database applications. Physicial Requirements and Working Conditions :
Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
Unique patient lifting / movement situations will be assessed on a case-by-case basis.
