Elite Home Health, LLC is a growing home health care agency based in Bedford, IN. We are seeking a passionate intake coordinator to join our team! The intake coordinator facilitates the intake referral process while coordinating care with the team and referral source.

Job Requirements and Responsibilities:

• Effectively evaluates incoming referrals for appropriateness for admission.
• Understands regulations and requirements for Medicaid home health admission.
• Obtains patient demographics and all required service information.
• Accurately fills out an intake information sheet on every referral.
• Accurately and completely enters the patient information into the EMR per agency process.
• Ensures seamless transition of patients to home care by providing direct oversight of patient education, preparation of home care, and coordination of care with multiple service providers.
• Provides education regarding available patient resources.
• Gathers, collates, and reports referral statistics including key referral trends.
• Maintains comprehensive working knowledge of the company's contractual relationships.
• Demonstrates teamwork and prompt regular attendance to provide quality services.

Qualifications/Skills:

• Minimum of two (2) years experience in Medicaid home health/waiver environment
• Knowledge of Medicaid home health and waiver program eligibility requirements
• Exceptional communication skills/telephone skills
• Demonstrates good negotiation and public relations skills
• Attention to detail
• Ability to multi-task with minimal supervision
• Proficient knowledge of Microsoft Office, especially Excel
• Valid Driver's License
• Reliable transportation
• Must pass a background check

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
