Position Summary

The PACE Medical Assistant manages the flow and operations of the PACE KC clinic. Coordinates services with primary care team and asks as a primary liaison. Provides care to the participants; implements provider orders; assists with developing appropriate plan of care; provides education to participants, care givers, families, and associates; and communicates participants condition to caregivers; primary care, and the interdisciplinary team (IDT).

Primary Functions

  • Helps patients by providing information, services, and assistance.
  • Maintains medical supplies inventory and performing preventive maintenance to keep medical equipment operating.
  • Verifies patient information by interviewing patient, recording medical history, and confirming purpose of visit by pt’s full name & documentation for verification at every visit, and recording this information in the electronic medical record.
  • Prepares patients for examination by performing preliminary physical tests; taking blood pressure, weight, and temperature; and reporting patient history summary.
  • Secures patient information and maintains patient confidence by completing and safeguarding medical records.
  • Counsels patients by transmitting physician’s orders and answering standard questions
  • Aids in scheduling appointments and ensures clinic is running on time.
  • Maintains safe, secure, and healthy work environment by establishing and following standards and procedures and complying with legal regulations.
  • Keeps equipment operating by following operating instructions, troubleshooting breakdowns, maintaining supplies, performing preventive maintenance, and calling for repairs.
  • Updates job knowledge by participating in educational opportunities and reading professional publications.
  • Serves and protects the practice by adhering to professional standards; facility policies and procedures; and federal, state, and local requirements.
  • Administers prescribed medications and maintains related medical records under supervision.
  • Administers medications using proper dosage, route, timing, and technique.
  • Perform medication reconciliation with the patient and record in the electronic medical record.
  • Conduct medical screenings such as depression, tobacco, and substance abuse.
  • Performs other duties as assigned.

Supervisory Duties and Responsibilities

Education, Certification, and Experience

Minimum

  • High school diploma or equivalent
  • Certified Medical Technician certification
  • Two (2) years of demonstrated experience working with the aging population.
  • Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
  • CPR/AED certified.

Desired

  • Bilingual
  • Previous long-term care experience, clinic manager experience, or Home Community Based Services

  • Ability to demonstrate patience, understanding, and sensitivity interacting with participants, families, visitors, and coworkers.
  • Excellent organizational skills with attention to detail and time management skills.
  • Effective analytical and problem-solving skills
  • Ability to work independently.
  • Effective oral and written communication skills
  • Knowledge of compliance and privacy laws and regulations related to health care systems.
  • Ability to review and analyze provider credentialing file for completeness, accuracy, issues, and changes.
  • Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care.
  • Proficient in the use of electronic medical records and Microsoft Office software
