General Description:
The Clinical Care Coordinator must have experience working with dialysis patients and be able to offer expert opinions for all aspects of dialysis. The Clinical Care Coordinator will provide ongoing education and training to new and established employees, patients, and various external clients. Act as an independent auditor for all units. Administers high quality program opportunities in a variety of settings that promote the health of patients, staff, and the reputation of Atlantic Dialysis Management Services and its affiliates.
Requirements:Position Qualifications:
Must have previous supervisory experience, dialysis experience.
Minimum Experience: Registered Professional Nurse with NYS RN License 2-year nursing experience with a minimum of 3 months dialysis experience
Req. Certification/Registration: BLS, ACLS
Required Education: Completion of accredited program for Registered Professional Nurse with New York State RN License
Education: Bachelor of Science in Nursing
Preferred Experience:
1-2 years of full-time chronic dialysis experience as an RN
1-2 years of full-time acute care nursing experience with cardiac monitoring
Required Qualifications: BLS, ACLS
Certified Dialysis Nurse/Certified Nephrology Nurse (may be obtained within first 6 months of employment
Current and valid driver’s license
Responsibilities:
1. Maintains established departmental policies and procedures, objectives, quality assurance/performance improvement program, safety, environmental, and infection control standards.
2. Able to perform all duties listed in the Registered Nurse job description.
3. Establishes and maintains standards of care. Provides clinical supervision for nursing and technical staff.
4. Assures quality patient care. Utilizes established quality care indicators as basis for identification of opportunities to improve care.
5. Develops specific staffing patterns for the Dialysis Center, Coordinates schedule for staff and shift times for patients.
6. Participates in the interview process.
7. Conducts staff performance evaluations and effectively recommends action to be taken regarding areas of probation and discharge.
8. Instructs and develops the potential of his/her staff members.
9. Provides effective communication.
10. Displays professional behavior and promotes own professional growth and development. Enhances professional growth and development through participation in education programs, current literature, in-service meetings, and workshops.
11. Promotes positive customer relations.
12. Maintains a safe environment.
13. Maintains CPR certification.
14. Ensures compliance with applicable Federal and State laws and regulations.
15. Performs other related duties as assigned or requested.
