As the Lead Nurse at Pacific Medical Resources, you will play a crucial role in overseeing the nursing staff and ensuring the highest quality of care for our patients. This position is a salaried position requiring a 40-hour work week. Additionally, you will be responsible for the on-call care phone in rotation with other Lead Nurses, typically for 7 days once a month, providing essential support care to our clients and team out in the field.

Key Responsibilities:

General:

• Participate in alternating after-hours on-call phone duty with other nursing staff to address urgent patient needs.
• Conduct 62-day supervisory visits to assess patient progress and ensure adherence to care plans.
• Perform thorough patient evaluations, spending up to 4 hours to assess medical needs and develop appropriate care strategies.
• Manage electronic medical records (EMRs) to maintain accurate and up-to-date patient information.
• Provide respite care for medically fragile patients, ensuring their comfort and well-being.
• Fulfill all duties outlined in the Nurse LVN/RN Description, serving as a role model and mentor for the nursing team.

When assigned in a Medically Enhanced Home:

• Coordinate and lead the care staff, ensuring smooth operations and efficient patient care.
• Act as a liaison between doctors and clients, facilitating effective communication and collaboration.
• Oversee nursing case management to optimize patient outcomes and promote efficient care delivery.
• Lead new client Medically Enhanced Assessments, gathering essential information and introducing them to our care programs.
• Deliver training sessions for staff and clients, promoting best practices and ensuring compliance with healthcare standards.
• Handle any other duties assigned by management to support the smooth functioning of our healthcare facility.

Requirements:

• Possess a clean driving record and have a reliable form of transportation.
• Hold a current California Registered Nurse license or LVN license in good standing.
• Maintain current Basic Life Support (BLS) certification.
• Successfully pass a comprehensive background check, physical and TB screening to ensure patient safety and welfare.
