Integrated Clinical Services provides quality health services for people who experience barriers to accessing care. The Student Health Center (SHC) Program is a unit within ICS. The vision of SHC is to facilitate access to comprehensive preventive, primary, and mental healthcare for Multnomah County school-age youth to keep them healthy and ready to learn. The Program operates in coordination with five school districts: David Douglas, Parkrose, Centennial, Reynolds and Portland Public Schools, and the Child and Adolescent Mental Health Program of Multnomah County Department of Community and Family Services. SHC clinics are located in 9 schools to provide easy access to health care for students. This position is responsible for developing and delivering activities designed to promote individual, family, and school community health by providing health education, care coordination and referrals to community and social support services. The position has a special focus on overdose prevention via health promotion and health education in the clinical setting and to provide school and community engagement. This position requires bilingual language skills in Spanish and English. *This position is open to the Health Department only.* *This position is a Limited Duration assignment that may last for up to 2 years or less depending upon the budget.* *This position requires Spanish speaking and Listening skills.* *This is a SHC (Student Health Center) year around position.* As the Community Health Specialist 2, other duties will include: Health Education/Promotion Work with the SHC Site Medical Director, Program Manager, Community Health Worker (CHW), Project Manager, Communications Specialist, and onsite clinic team to plan and deliver interactive health education classes and presentations for a variety of groups in the school community. Present culturally and age appropriate, department-approved health education and information with students, families and groups (written and verbal). Provide education that assists patients in identifying goals for behavior change, to include information about the dangers and risks of drug use, effects of drug use to a developing/school-age mind and body, warning signs of a potential overdose, the use and purpose of Naloxone, and how to respond to a potential overdose. Participate in the development of systems that encourage strong client participation and increase client’s ability to learn and apply health information to make good decisions. Use Spanish and English language skills in service delivery to and for clients. Direct Client Services and Documentation Respond to direct referrals from medical providers. CHWs receive referrals for care coordination, individual and family support, health promotion, and referral to community and social support services based on chief complaints in Epic under the Person Centered Medical Home Umbrella. Document services with patients in Epic via interim notes, telephone encounters and work referral reports. Each encounter must be documented in accordance with established work-flows to ensure continuity of touches and care. School and Community Engagement Act as conduit between the SHC and schools to increase awareness of health centers and generate positive communications (print, publications, etc). Assist with facilitation and support of Student Health Action Councils. Provide advocacy to the Councils, listening for changes within that location and student body for opportunities to grow programs and services to address needs. Act as a reliable source of information to the Councils so the Councils can provide to the student body current and accurate information based on science and facts, while remaining open to hearing cultural or historic needs and concerns. Ensure that the community and school partners are aware of and know how to help students and families to access services. Provide a variety of ways to address concerns and options for client/student decisions. Other: Must have valid CPR / BLS certification upon hire OR within 30 days of hire. *Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experience that are relevant for this position.
