Serenity Health Care Services is Hiring a Care Manager Near Bronx, NY

Job Overview:

Seeking excellent communicators who are compassionate and looking to make a difference. In this role, you would be helping members of the community access healthcare and support services according to each client's needs. Health home care managers construct individualized and comprehensive care plans by thoughtfully assessing and identifying health and wellness barriers. A home health care manager's primary focus is to support the maintenance and promotion of their client's wellness by aiding public access to critical health care services.

Pay is determined by the number of cases you decide to accept.

Ideal Candidate:

  • Be reliable & empathetic
  • Experience with care management, HCBS, or NY Waiver Program
  • Fluency in languages other than English

Requirements:

  • Available for part-time or per-diem work
  • Perform home visits (case-related documentation can be done remotely)
  • Be a NYS resident
  • Possess a Bachelors degree
  • Excellent verbal and written communication skills
  • Technology proficient (Use of Microsoft Office & other documentation programs required)
