Responsibilities:

  1. Patient-Centric Care Coordination:
    • Oversee patient care personnel, ensuring seamless coordination among all disciplines, physicians, and caregivers.
    • Supervise the planning, coordination, and delivery of patient/family care with a focus on maintaining compliance with Medicare and Medicaid standards.
  2. Clinical Management with Medicare/Medicaid Expertise:
    • Manage clinical activities, leveraging over 35 years of expertise in patient assessments, care plan development, and service level determination.
    • Implement, coordinate, maintain, and evaluate all care plans, ensuring alignment with Medicare and Medicaid requirements.
  3. Supervision and Performance Excellence:
    • Directly supervise clinical staff, utilizing your experience to evaluate their performance in alignment with our established job goals and requirements.
    • Play a key role in the recruitment, interviewing, selection, and orientation of team members, contributing to our legacy of dedicated professionals.
  4. Legacy, Compliance, and Certification:
    • Uphold our legacy of quality care by coaching staff, recommending in-service education programs, and ensuring adherence to our internal policies and standards.
    • Discuss operational issues, keeping the team updated on new/changed regulations, and reviewing records/documentation to maintain our long-standing commitment to regulatory and in-house compliance, particularly with Medicare and Medicaid guidelines.
  5. Community Engagement and Growth:
    • Participate in community engagement activities and foster relationships to ensure service quality and effective care coordination.
    • Contribute to the growth and differentiation of our services in the Tri-County area, highlighting our Medicare and Medicaid certified status.

Qualifications:

  • Bachelor's degree in Healthcare Administration (Preferred).
  • Proven experience in home healthcare administration, ideally with a focus on serving the Michigan Tri-County area.
  • In-depth knowledge of regulatory compliance, healthcare policies, and Medicare/Medicaid guidelines.
  • Exceptional communication, leadership, and organizational skills.
  • Ability to mentor and coach clinical staff effectively.
  • Proficiency in relevant software and documentation tools.
