We have open positions and are looking for the very best to support clients in their homes in the Metro Detroit Area, specifically Southern Oakland and Wayne County.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and/or certified by successfully completing the National Certification examination.

• One year of COTA experience in a Skilled/Clinical environment
• Current State of Michigan Certified Occupational Therapist Assistant (COTA) license in good standing
• Graduate with Bachelor of Science or Master of Science Degree in Occupational Therapy.
• Able to provide clinical services in the home and able to instruct/teach patient and caregivers.
• Able to take instruction from OTR supervisor to provide services to the patient according to regulatory guidelines and Physician Supervised care plan
• Current & Valid Driver’s License with reliable and insured transportation
• Current CPR Certification
• Current Negative TB test/negative chest x-ray
• Functioning Electronic Device with cellular service so you can chart in the field
• Knowledge and experience working with home health care specific Electronic Charting
• Work requires knowledge of the Homecare Industry, Medicare/Medicaid/CMS/State of Michigan regulations
• Freedom from illegal use of drugs. Freedom from use and effects of the use of drugs and alcohol in the workplace.
• Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a healthcare-related setting are ineligible for employment in the position.

Essential Functions:

• Provides Occupation Therapy services under the supervision of an Occupational Therapist.
• Care for multiple patients throughout the day in their individual home environments
• Follow care plan and administer type and frequency of therapy treatments prescribed;
• Report significant findings to OTR Case Manager, Physician, and Clinical Manager
• Follow Physician and OTR orders in a timely, complete and accurate manner
• Ensure the safety of the treatment procedures, patient and environment at all times
• Teach the patient and/or family members about new diagnosis, care, and support skills
• Submit documentation in a timely, complete, and accurate manner
• Attend in-services and meetings as required
• Maintain HIPAA compliance and patient confidentiality at all times
• Position requires 100% travel to our patient’s home
