Program Summary

The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The Network consists of three (3) federally qualified health centers (FQHC) in Philadelphia:

  • Abbottsford Falls Family Practice & Counseling (Northwest Philly);
  • Stephen & Sandra Sheller 11th Street Family Health Services of Drexel University (North Philly); and
  • Health Annex (Southwest).

Today the FPCN serves more than 20,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.

Position Summary

The Director of Perinatal Services (DPS) is a Certified Nurse-Midwife (CNM) directs health services for the prenatal and postpartum services at the Family Practice & Counseling Serivice’s primary care sites (currently 3) as well as all services as the upcoming Family Health & Birthing Center (“Birth Center”) expected to open late 2024 in SW Philadelphia. The DPS coordinates care between the Birth Center and referral hospitals in accordance with guidelines of the American College of Nurse-Midwives (ACNM), American Association of Birth Centers (AABC), the Commonwealth of Pennsylvania Department of Health Birth Center Regulations, and the protocols/practice guidelines of the Family Practice & Counseling Center (FPCN).

Essential Duties and Functions

CNM duties

  • See prenatal and postpartum patients at the network sites as needed.
  • Attend births at the Birth Center.
  • Attend births at Hospital of University of Pennsylvania to continue to developing string relationships with their L&D and NICU teams.

This position preliminarily supervises clinical functions of the start-up peri-natal team including CNMs, Nurse Practitioners (CRNPs), Lactation Consultants, Doulas, and Registered Nurses (RNs) until the birth center opens, and then a Lead Midwife is hired. Duties include:

  • Hire, orient, and evaluate clinical staff as necessary in collaboration with Human Resources and the Practice Manager.
  • Coordinating monthly clinical schedules.
  • Support continuing education.
  • Assist in meeting productivity expectations.
  • Coordinate with contracted agencies providing doula and home visiting care.

Provide clinical leadership and oversight for CNMs and CRNPs including but not limited to:

  • Develop, implement, and share clinical and administrative policies and protocols in collaboration with the Executive Leadership Team (XLT) and the Clinical Advisory Committee (CAC).
  • Follow-up on patient care concerns/issues in conjunction with Practice Manager and Center Directors.
  • Promote Quality Improvement processes, power cycles, and support network improvement initiatives and quality measure goals.
  • Work with Executive Clinician, Director of Primary Care, and Clinical Nurse Specialists to ensure safe, up-to-date, evidence-based practice across the network.

Coordinate and serve on the CAC.

Assist in coordination of clinical education experiences for students and volunteers.

Advise XLT on the development and administration of the annual budget, fee schedules, and third-party insurance negotiations.

Develop new clinical programs in conjunction with the XLT and CAC.

Collaborate with the Integrated Behavioral Health Specialists, Out-Patient Mental Health Services, and Medical-assisted Therapy teams.

Liaison with Collaborating Hospital(s)

  • Participate in medical staff activities at the collaborating hospital including meetings and huddles.
  • Hold a joint appointment with admission privileges at both the Birth Center and the collaborating hospital.
  • Meeting with hospital leadership to ensure care coordination across various care delivery locations.

Represent the Birth Center at community events, fundraising activities, and in the media as needed.

Perform or delegate other duties as assigned where appropriate.

Reports to Executive Director and/or Executive Clinician.

Required Education:

  • Master’s Degree in Nursing or a Healthcare Management field

Required Experience:

  • Minimum 5 years as a CNM including Out-of-Hospital/Birth Center experience; work with low-income communities.

Required Qualifications:

  • Current Pennsylvania CNM license
  • Current nationally recognized CNM certification
  • Must have a valid driver’s license and transportation to travel between sites.
  • Current CPR Certification
  • COVID Vaccinated

This currently does not include boosters.
