Job Summary
We are seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. The ideal candidate will evaluate patients, develop individualized treatment plans, and provide high-quality physical therapy services to help patients regain mobility, strength, and independence. This position offers the opportunity to work in a collaborative skilled nursing and subacute rehabilitation environment. New graduates are encouraged to apply.
Key Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Implement, monitor, and modify physical therapy interventions based on patient progress.
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient care.
- Establish realistic rehabilitation goals and monitor patient outcomes.
- Maintain accurate and timely clinical documentation in compliance with state, federal, and facility regulations.
- Educate patients and families on treatment plans, home exercise programs, and injury prevention.
- Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs).
- Participate in interdisciplinary care planning meetings and MDS processes.
- Ensure compliance with HIPAA, safety standards, and facility policies.
Qualifications
- Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited program.
- Current Arizona Physical Therapist (PT) License or eligibility for Arizona licensure.
- Current CPR Certification.
- Strong communication and interpersonal skills.
- Knowledge of HIPAA regulations and Skilled Nursing Facility (SNF) requirements.
- Ability to work collaboratively within a multidisciplinary healthcare team.
Preferred Qualifications
- Experience in Skilled Nursing Facilities (SNF), Subacute Rehabilitation, Long-Term Care, or Acute Care settings.
- Experience with patient evaluations, treatment planning, and clinical documentation.
- Familiarity with MDS assessments and interdisciplinary care planning.
