We are seeking a Physical Therapist (PT) for a PRN opportunity at Nellas Autumn Lake. Join a collaborative rehabilitation setting where you can provide skilled, resident-focused therapy designed to improve mobility, strength, balance, and everyday function.

What You'll Do

• Perform physical therapy evaluations and develop individualized plans of care
• Provide treatment addressing strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress
• Modify treatment plans based on clinical needs and response to therapy
• Educate residents and caregivers on exercises and safe mobility techniques
• Maintain clear and timely clinical documentation
• Coordinate care with PTAs, nursing staff, and other rehabilitation professionals

Qualifications

• Active West Virginia Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and teamwork skills
• Skilled nursing or post-acute rehabilitation experience preferred, but not required
• New graduate Physical Therapists are welcome to apply
