Job Opportunity: Social Worker
Location: Prescott, AZ 86301-1641
Shift Schedule: 8-hour Day shifts
Assignment Duration: 15 weeks
Pay Rate: $40–$45 per hour (W2)
Radius Rule: 50 miles

This is a 15-week assignment for an experienced social worker to provide comprehensive support in a variety of inpatient and emergency settings. The ideal candidate will have strong skills in discharge planning, mental health assessments, and crisis intervention.

Unit Overview:

• Department: Social Work
• Cost Center: Social Work
• Caseload: 12-16 patients per day (varies based on patient needs)
• EMR System: Cerner
• Areas Covered: Inpatient, Emergency Department, Family Birthing Center, ICU

Requirements:

• Licensure: LMSW or LCSW (Arizona license required)
• Certifications: BLS not required
• Experience: Experience with mental health assessments, discharge planning, and working with diverse patient populations is essential.
• Skills Required:
• Social work mental health assessments
• Discharge planning
• Crisis intervention (including Title 36 petitions)
• SDOH (Social Determinants of Health) consultations
• Maternal and child assessments
• Knowledge of homelessness and substance abuse issues
• DCS and APS reporting

Shifts & Scheduling:

• Standard Hours: 8:00 AM – 4:30 PM
• Weekend Requirement: May be required once every 3-4 weeks (if at all)
• Holiday Requirement: None

Additional Details:

• Dress Code: Business casual attire
• Department-Specific Orientation: 1-2 weeks with Social Worker Manager
• Discharge Planning: Varies by patient and case needs

Clinical & Professional Environment:

This position involves working closely with other healthcare professionals to manage complex social issues within a hospital setting. As a social worker, you will support patients and families through emotional, mental, and logistical challenges that arise during hospitalization, and you will collaborate with multidisciplinary teams to ensure that each patient has the resources they need for a successful discharge and recovery.
