What are the responsibilities and job description for the Social Worker position at We-Care Staffing MD LLC?
· Work with high-risk patients in both inpatient and outpatient settings.
· Help patients get the care and support they need—from planning services to connecting them with social services, mental health providers, substance abuse programs, and health care.
· Track patient progress and adjust plans as needed.
· Make sure patients only get necessary referrals and use hospital resources efficiently.
· Work as part of a team of doctors, nurses, and social workers to improve patient outcomes.
· Your goal: help patients get better care, improve quality of life, and reduce unnecessary costs.
For Social Workers (MSW):· Master's Degree in Social Work (MSW)
· Licensed as LMSW or LCSW (valid in any U.S. state or territory)
· BLS certification (American Heart Association)
· At least 3 years of clinical care experience
· Experience in discharge planning, case management, or utilization review
· Familiarity with electronic health records
· At least 1 year of travel/contractor experience
