MDS Nurse RN/LPN
Job Summary:

Responsibilities:
- Conduct and coordinate the development and completion of the resident assessment
- Conduct resident assessments in accordance with current rules, regulations, and guidelines
- Implement resident CAA's
- Participate in facility survey readiness and throughout the survey
- Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
- Current unencumbered State of Massachusetts RN or LPN License
- Two years of MDS and PPS experience
- Care Planning experience
- Competitive wages: $45 - $55 HOURLY
