MDS Nurse RN/LPN

Job Summary:

Join our nursing facility, Winchester Rehabilitation and Nursing Center, as an MDS Nurse RN/LPN! As a valued member of our team, you will play a crucial role in providing high-quality care to our residents. Responsibilities:

• Conduct and coordinate the development and completion of the resident assessment
• Conduct resident assessments in accordance with current rules, regulations, and guidelines
• Implement resident CAA's
• Participate in facility survey readiness and throughout the survey
• Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
  
  
  

Requirements:

• Current unencumbered State of Massachusetts RN or LPN License
• Two years of MDS and PPS experience
• Care Planning experience
