What You’ll Be Doing
- Deliver direct and indirect patient care based on nursing plans and facility policies.
- Administer prescribed medications, treatments, and therapies.
- Monitor patient vitals, assess physical and mental status, and document changes.
- Collaborate with healthcare teams to develop and implement personalised care plans.
- Facilitate seamless patient admissions, transfers, and discharges.
- Ensure accurate, complete, and up-to-date nursing documentation.
- Maintain a clean and safe environment for patients and staff.
- Provide emotional support and education to residents and their families.
- Follow strict infection control and safety protocols.
What You’ll Need to Succeed
- Active NY LPN License.
- Current BLS Certification.
- At least 1 year of experience in long-term care as an LPN.
