Pay Rate:
Range $1480 to $1520 Weekly Gross pay based on a 40 hour work week (Includes non-taxable
travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage
Reimbursement: $0.49/mile for distance
covered between patient home appointments
Unit: Home Health
Shifts available: Days
Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
- LPN/LVN experience: 2 years - Required
- Recent/Current Home Health Nursing
experience: 1 year within the past 3 years - Required
- Home Care Home Base (HCHB)
charting experience - Required
- Familiar with OASIS
- Must have valid Driver’s License
and Personal Auto Insurance
- Must be Covid Vaccinated or
provide medical or religious exemption
Education, License
and Certification Requirements:
- LPN License for State of Oklahoma or Compact -
Required
- BLS Certification from AHA or
American Red Cross - Required
Job Description:
- Provide and coordinate care for
assigned patients in comfort of their homes.
- Implement care plans under supervision.
- Support patient recovery and
ability to stay at home - enabling them to spend quality time with their
loved ones and doing the activities they enjoy
- Empower patients and families to
reach healthcare goals by educating them on disease management
- Under supervision, provide nursing care and services
within the standards and scope of nursing practice as defined within the
state Nurse Practice Act regulations, policies, procedures, and protocols,
as well as facility and corporate policies and procedures.
Ensure patients’ status is monitored on an ongoing basis and pertinent information is gathered and documented. - Notify RN or Nurse Manager or IDT team member of significant
changes in status
- Document and complete charting
for all patient visits within 24 hours.
- Territories to cover: Southwest Region
