Our campus offers residents delicious dining and a continuum of healthcare including an on-site ambulatory clinic, Assisted Living, Memory Care and Skilled Nursing. Our internal medicine clinic is a patient-centered clinic focused on providing comprehensive, compassionate, and evidence-based care to adult patients. We treat a wide range of chronic and acute medical conditions, with an emphasis on preventive health and long-term wellness. Understand the importance of resident rights in healthcare as well as the community as a whole, and advocate for operations, processes, and decisions that are in residents’ best interests. Clinical Leadership: Provide medical oversight for residents within the SNF and clinic, including the diagnosis, treatment, and management of acute and chronic conditions. Develop and implement care protocols, clinical guidelines, and best practices to optimize patient outcomes and ensure high standards of care. Collaborate with nurse practitioners, nursing and clinical staff, and other healthcare professionals to provide comprehensive and coordinated care. Serve as a clinical resource and mentor to nursing and clinical staff, promoting professional development and continuous learning. Conduct regular patient rounds, review care plans, and provide consultation on complex medical Serve on the Quality Assessment and Assurance Committee , which encompasses Infection Control, Ethics, Policy and Procedure Review, Physician, and Pharmacy. Participate in quality assurance and performance improvement (QAPI) programs Administrative & Management Duties: Lead the medical operations of the SNF, MSC, CS, Clinic, and all medical care components associated with the community, ensuring compliance with all state and federal regulations, including CMS guidelines. Work closely with the Executive Director, Healthcare Administrator, Director of Nursing, and other leaders to develop and implement strategic goals, policies, and programs for the facilities and clinic. Manage relationships with external providers, including hospitals and specialists, to facilitate seamless transitions of care and strengthen partnerships. Engage in community outreach and wellness programs to promote preventive care and health education for residents. Attend and contribute to interdisciplinary team meetings, case conferences, and leadership meetings. Supervise other physicians and nurse practitioners who provide care for residents in the community and review applications for Physician Credentialing Patient Care & Advocacy: Engage directly in patient care by conducting medical evaluations, prescribing medications, and developing treatment plans for clinic patients. Work closely with residents and their families to educate and inform them about treatment options, care goals, and advance care planning. Serve as a key contact for medical emergencies and critical decision-making situations, ensuring resident safety and quality care. Perform other duties as assigned About you: Minimum of 1 year of experience providing patient care. Doctor of Medicine (MD) degree from an accredited college or university. Valid Medical License from the state Medical Board. Current Drug Enforcement Agency (DEA) registration Proven ability to establish therapeutic relationships with diverse age groups and medical needs, demonstrating patience, care, understanding, and effective problem-solving skills. Proficiency with computer systems and electronic health records. Working knowledge of regulations regarding operation of a healthcare clinic, HIPAA, Medicare/Medicaid and OSHA safety regulations. Effective organizational skills to manage schedules, coordinate services, and ensure that all necessary documentation and paperwork are completed accurately and on time.
