We are seeking an experienced MDS Coordinator to oversee the RAI process, ensuring timely and complete MDS assessments, Care Area Audits (CAAs), and Interdisciplinary Care Plans.

- Monitoring and guiding assessment completion
- Supervising input and transmission of MDS assessments to the state, ensuring validation
- Facilitating care plan conferences and monthly Quality Meeting (QM) sessions
- Managing the care planning process
- Licensed RN or LPN with an active, unencumbered license in Ohio
- Degree in Nursing from an accredited institution
- Minimum of 3 years’ experience as an MDS Coordinator in Long Term Care
- RAC-CT certification preferred
- Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement
- Excellent communication, critical thinking, and organizational skills
- Knowledge of federal, state, and local regulations governing long-term care
