POSITION DESCRIPTION:
The Case Manager under the direction of the Program Director is responsible for the ongoing concrete needs assessment, implementation, support, information, referral, and advocacy for individuals who are in the Providence House Women’s Justice Program (WJP). Keeping clear and accurate files and records is essential to the position.
Responsibilities:
Client/Service Delivery
- Responsible for assessing the concrete needs of the residents
- Meet bi-weekly with residents to discuss progress on concrete goals, development of life skills and provide a supportive environment for the discussion of life issues which includes housing entitlements, employment, and education.
- Arrange meetings with pertinent staff that will assist residents in securing housing, employment, and other services:
- Prepare residents to negotiate with agencies, i.e., housing, employment, HRA, and education.
- Provide employment and job readiness assistance
- Enter and maintain current client information in AWARDS case management system
- Maintain a current list of community resources and ensure that residents receive needed services.
- Escort residents to appointments as needed
- Complete self-sufficiency assessments and document progress notes bi-weekly
- Maintain accurate case records and prepare required forms and reports for outside agencies.
- Participate in individual supervisory sessions and case reviews with the Program Director.
- Attend all mandatory training, workshops and meetings as required.
- Meet with program staff, core community and other pertinent staff on a regular basis to discuss matters of mutual concern.
- Assist with behavior management issues as it relates to following up on resident’s compliance with program regulations
- Performs other duties as assignments by the Program Director.
Team/Staff Relations
- Work as a Providence House team member with all staff, volunteers, and interns to implement the mission and core values of the organization.
- Attend regular internal and external meetings.
- Work as part of a team to plan and develop resident programs.
Administrative
- Keep accurate, up-to-date documentation on all residents, including but not limited to, treatment plans and case notes, etc.
- Complete all paperwork in a timely and accurate manner
- Assist with any and all data collection systems.
Travel
- Requires travel with residents to outside agencies when necessary.
- Requires travel to Providence House residences as well as meetings, workshops, conferences with outside organizations.
SANCTUARY MODEL:
- Effectively use and practice Sanctuary Models tools, safety plans, community meetings, team meetings, red flag reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
- Promote and model the core values and mission of Providence House (compassion, non-violence, hospitality, and community) and the seven Sanctuary Model commitments to create a non-violent and trauma sensitive culture.
- Integrate the Sanctuary Model SELF (safety, emotional management, loss, and future) framework into daily practice to ensure quality care and an effective work environment.
QUALIFICATIONS:
- B.A. Degree in Social Work or related field.
- Experience working with homeless and/or mentally ill population.
- Excellent verbal and written communication and interpersonal skills and the ability to work independently.
- Competent technology management skills, including proficiency with using Microsoft Office; client database management.
- Compatible philosophy with Providence House and Sanctuary paradigm.
WORK SCHEDULE:
The Case Manager position is a full-time, non-exempt position. The hours worked will be Tuesday through Saturday from 9 AM to 5 PM. The office location for this position is Brooklyn, New York.
SALARY AND BENEFITS:
The salary for this position is $50,000.00 annually. Paid time off benefits include: 14 paid holidays, 3 weeks of vacation (with additional accruals based on agency tenure), 4 personal days (1 in recognition of your birthday which can be used any time), 12 sick days and early closure on Fridays at 12pm proceeding Monday holidays.
