Key ResponsibilitiesAssessment & Planning: Evaluate client physical, mental, and emotional needs; develop individualized care/treatment plans.Care Coordination: Act as a central point of contact, facilitating communication between clients, families, doctors, and other providers.Resource Navigation: Connect clients with appropriate social services, community resources, and support programs.Advocacy: Champion clients' best interests, ensuring they receive safe, timely, equitable, and cost-effective care.Monitoring & Evaluation: Track case progress, review treatment effectiveness, and determine case closure.Documentation: Maintain accurate, up-to-date records and case files.
