A Case Manager job involves assessing client needs, creating personalized care plans, coordinating healthcare/social services, advocating for patients, and monitoring progress to ensure holistic well-being, improved outcomes, and efficient resource use, bridging gaps between clients (e.g., chronic illness, disabilities, elderly) and complex support systems. Key duties include needs assessment, care planning, provider communication, family liaison, resource navigation, and record-keeping, requiring strong advocacy, problem-solving, and organizational skills. Key ResponsibilitiesAssessment & Planning: Evaluate client physical, mental, and emotional needs; develop individualized care/treatment plans.Care Coordination: Act as a central point of contact, facilitating communication between clients, families, doctors, and other providers.Resource Navigation: Connect clients with appropriate social services, community resources, and support programs.Advocacy: Champion clients' best interests, ensuring they receive safe, timely, equitable, and cost-effective care.Monitoring & Evaluation: Track case progress, review treatment effectiveness, and determine case closure.Documentation: Maintain accurate, up-to-date records and case files. Essential Skills & QualificationsStrong communication (verbal & written) and interpersonal skills.Excellent problem-solving, organizational, and delegation abilities.Ability to work independently and collaboratively.Often requires a Bachelor's degree (e.g., Nursing, Social Work) and relevant licensure (RN, LCSW).Familiarity with healthcare systems, insurance, and community resources.

Salary : $60,000 - $70,000
