Job Title: Case Manager (Contract)Location: Brooklyn, NY 11207⏰ Schedule: Tuesday – Saturday, 9:00 AM – 5:00 PMPay: $22.00 – $24.00/hour (Contract)Type: ContractPosition SummaryWe are seeking a dedicated Case Manager (Contract) to provide supportive services to adults (ages 25–91) with mental health and substance use needs residing in permanent housing. The Case Manager will play a key role in helping tenants maintain housing stability, access community resources, achieve personal goals, and promote overall wellness.Key ResponsibilitiesManage a caseload of 15–18 tenants, developing individualized support plans.Provide crisis intervention, problem-solving, and assistance with daily living needs.Accompany tenants to medical, psychiatric, and public benefit appointments.Conduct home/hospital visits, apartment inspections, and assist with tenant moves.Facilitate recreational, educational, and skill-building activities to build community inclusion.Act as liaison to community-based services and advocate for tenants while encouraging self-advocacy.Document services and participate in supervision, team meetings, and case conferences.Support Facilities Management staff to ensure compliance with OMH apartment standards.Provide on-call crisis support as needed.QualificationsRequired:Bachelor’s degree in Human Services or related field.Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).Preferred:2 years’ experience working with individuals with mental illness, substance use, or history of homelessness.Background in supportive housing, case management, or community outreach.Group work and tenant engagement experience.Bilingual (English/Spanish) a plus. Company DescriptionPride Global offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts with preferred vendors.
