Job Overview
We are seeking a licensed Physical Therapist to join our Home Health team in Clovis, NM. This role focuses on improving patient mobility, restoring function, reducing pain, and preventing injury through personalized care plans and therapeutic interventions.
Key Responsibilities
- Perform initial physical therapy evaluations and complete admission documentation (including OASIS when applicable)
- Develop individualized treatment plans and home exercise programs with measurable goals
- Establish visit frequency and duration (typically 2-3 visits per week over a 60-day certification period)
- Monitor patient progress and adjust treatment plans as needed
- Coordinate care with nursing staff and communicate patient updates
- Contact physicians for status changes and obtain necessary orders
- Perform supervisory visits for CNAs when required
- Educate patients and families on safety, fall prevention, and home ergonomics
- Assist with obtaining adaptive equipment and insurance authorizations
- Maintain accurate and timely documentation (daily and weekly updates)
- Perform recertifications, discharges, and wound care as needed
- Support staff training, in-services, and occasional marketing initiatives
- Active Physical Therapy License (Required)
- Minimum 1 year of patient monitoring experience (Preferred)
- Home Health experience (Strongly preferred)
- Strong communication and organizational skills
