What are the responsibilities and job description for the Community Health Worker Navigator position at Northwest Family Services?
Provide individuals with culturally appropriate information for the purpose of reducing barriers to low-cost or no-cost breast and/or cervical cancer screening services.
What you’ll do:
· Provide screening and health system services as well as breast and/or cervical cancer screenings to program participants.
· Enroll a minimum of 80 individuals in the program for each of the Clackamas, Multnomah and Washington counties.
· On an annual basis provide at least three culturally appropriate outreach and education screening services for breast and cervical cancer.
· Conduct intake, assessment, and orientation for ScreenWise services.
· Assist individuals diagnosed with cancer to enroll in Project Access Now (Tri-County) or Project Access Network (Marion and Polk Counties).
· Provide care coordination and navigation to individuals who have either individual or employer health insurance.
· Schedule screenings with network ScreenWise providers.
Education / Experience
· Bilingual, bi-cultural English and Spanish required.
· A high school diploma or equivalent is required.
· Community Health Worker certification or experience is preferred.
